You will fill out this form below to start that process. 

1. Fill out the form via QR code/In person Form
2. Drop the item you plan to customize in the basket at Bohemia Beauty Lounge
3. I will respond to your contact info with a confirmation of design/invoice
   1. Invoices must be paid within 24 hours or blank items will be restocked to the floor
4. Embroidery happens
   1. Current Turnaround time is 1-2 Weeks
   2. Rush/Same Day orders are welcome but will result in a rush fee
5. Pick up your item from Stitched By Nik inside Bohemia Beauty Lounge
